An open thoracotomy surgical technique for aortic valve replacement is disclosed. A surgical incision (200) is made in one of several locations such as a midline sternotomy incision, a small anterior right or left thoracotomy, a mini-thoracotomy, a posterior thoracotomy, a suprasternal supra-clavicle approach, or through port sites (mini incisions) over the chest wall. The pericardium is then opened. The faulty natural aortic valve, next to the left ventricle (2), is excised and removed. A fastener driving tool with a cuff (19) attached is inserted through the incision (200). The knob (36) is rotated while holding handle (35) to deploy the fasteners through the cuff (19) to secure it. Finally, a prosthesis valve body (20) is attached to the cuff (19) before closing the incision (200).
L'invention a trait à une technique chirurgicale à thorax ouvert aux fins du remplacement d'une valvule sigmoïde. On pratique une incision (200) sur un emplacement sélectionné parmi plusieurs autres, en incisant, par exemple, la partie médiane du sternum, en réalisant une petite thoracotomie antérieure droite ou gauche, une mini-thoracotomie, une thoracotomie postérieure, ou en opérant par approche suprasternale sus-claviculaire ou encore en incisant en divers points d'accès (mini-incisions) de la paroi thoracique, après quoi l'on ouvre le péricarde. On excise et retire alors la valvule sigmoïde naturelle défaillante, proche du ventricule gauche (2). On insère, à travers cette incision (200), un outil permettant la mise en place d'une attache, pourvu d'un bracelet (19). Par rotation du bouton (36), le manche (35) étant maintenu, on déploie les attaches à travers le bracelet (19) afin de fixer celui-ci et l'on raccorde enfin un corps de valve prothétique (20) au bracelet (19) avant de refermer l'incision (200).
