When performing an end-to-side anastomosis connecting the end (10) of a first vessel (9) to the edge region (12) around an opening in the side wall of a second vessel (11), an instrument (1) is used having a circumferential stapling anvil (3) secured to an anvil tube (2) accommodating the first vessel (9), the end of which is everted about the anvil (3) with the intima of the end region (10) facing the intima of the edge region (12), when the instrument (1) with the first vessel (9) in place has been inserted into the opening in the second vessel (11). Then, a clamping tube (5) is advanced towards the anvil (3), clamping the end (10) and edge region (12) together against the anvil (3). In the next step, a number of stapling plungers (6) carrying staples (7) are advanced towards the anvil (3), the latter having staple- bending surfaces (4), thus stapling the two vessels together in a manner not shown. Finally, the clamping tube (5) and the stapling plungers (6) are withdrawn, and the whole instrument removed from the anastomosis.
Lorsqu'on effectue une anastomose extrémité-côté reliant l'extrémité d'un premier vaisseau (9) à la région périphérique (12) entourant une ouverture dans la paroi latérale d'un deuxième vaisseau (11), on utilise un instrument (1) comportant une enclume agrafeuese circonférentielle (3) fixée à un tube d'enclume (2) logeant le premier vaisseau (9), dont l'extrémité est retournée sur l'enclume (3), l'intima de la région d'extrémité (10) faisant face à l'intima de la région périphérique (12), alors que l'instrument (1) a été inséré, le premier vaisseau (9) étant en place, dans l'ouverture du second vaisseau (11). Ensuite, un tube de serrage (5) est avancé vers l'enclume (3), serrant l'extrémité et la région périphérique (12) ensemble contre l'enclume (3). Lors de l'étape suivante, un certain nombre de plongeurs agrafeurs (6) portant des agrafes (7) sont avancés vers l'enclume (3), celle-ci présentant des surfaces de pliage des agrafes (4), les deux vaisseaux étant ainsi agrafés ensemble, cet agrafage n'étant pas représenté ici. Enfin, le tube de serrage (5) et les plongeurs agrafeurs (6) sont retirés, et la totalité de l'instrument est retiré de l'anastomose.
