This invention is a pericardial centesis apparatus (100) and method for accessing the pericardial space. The invention consists of inserting a percutaneous tube (104) whose tip has a hole (123) which is positioned over and contacts the anterior pericardium. Introducing a vacuum within said tube (104) forms a pericardial bleb within that hole. A guided needle (124) within the tube is advanced to puncture the pericardial bleb while avoiding contact with the epicardium. A hollow filament or electro-cardial lead or flexible guide wire (114) within said needle can then be advanced into said pericardial cavity. The guide wire (114) may be used to guide an intra-pericardial catheter into the pericardial space for injection or infusion of selected therapeutic agents into the pericardial space to treat various heart and blood vessel diseases. Controlled drug release materials can be injected through said needle (124) for the slow and/or sustained delivery of said therapeutic agents into said pericardial cavity.
Appareil et procédé permettant d'accéder à l'espace péricardique .Cette invention se rapporte à un appareil de ponction péricardique (100) et à un procédé permettant d'accéder à l'espace péricardique. L'invention consiste à insérer un tube percutané (104) dont l'extrémité comporte un orifice (123) placé au-dessus du péricarde antérieur et venant en contact avec ce dernier. L'introduction d'un vide dans le tube (104) entraîne la formation d'une bulle péricardique à l'intérieur de cet orifice. On fait avancer une aiguille (124) guidée dans le tube afin de perforer la bulle péricardique tout en évitant de venir en contact avec l'épicarde. Puis on fait avancer dans la cavité péricardique un filament creux ou fil électro-cardial ou fil de guidage flexible (114) se trouvant à l'intérieur de cette aiguille . Le fil de guidage (114) peut être utilisé pour guider un cathéter intra-péricardique dans l'espace péricardique et permettre l'injection ou la perfusion dans l'espace péricardique d'agents thérapeutiques sélectionnés et de traiter divers maladies cardiaques et cardiovasculaires. Des médicaments à libération régulée peuvent être injectés par l'intermédiaire de l'aiguille (124) pour administrer des agents thérapeutiques de manière lente et/ou prolongée dans la cavité péricardique.
