A method for identifying and classifying various types of oversensing in implantable medical devices (IMDs), such as implantable cardioverter defibrillators (ICDs), to assist a physician in choosing corrective action to reduce the likelihood of oversensing and inappropriate therapy delivery. Far- field electrogram (EGM) signals are analyzed to detect the occurrence of R- waves, and the result is compared to the number and pattern of R-waves sensed by the IMD and indicated on the marker channel. A marker channel with more sensed R-waves than indicated by analysis of the far-field EGM indicates the presence of oversensing, including double-counting of R-waves, T-wave oversensing, lead malfunction or failure, poor lead connections, noise associated with electromagnetic interference, non-cardiac myopotentials, etc.. Identification of the type of oversensing may be determined by analysis of the number and pattern of marker channel sensed R-waves with respect to the timing of the R-waves detected from the far-field EGM.
L'invention porte sur un procédé d'identification et de classification de différents types de surévaluation dans des dispositifs médicaux implantables (IMD), tels que des défibrillateurs à synchronisation automatique implantables (ICD), afin d'aider un médecin à choisir une action permettant de réduire la probabilité de surévaluation et d'une thérapie inadaptée. Des signaux d'électrogramme à champ lointain (EGM) sont analysés afin de détecter l'apparition d'ondes R, et le résultat est comparé au nombre et au schéma d'ondes R détectées par les IMD et indiquées sur la canal marqueur. Un canal marqueur possédant plus d'ondes R détectées que celles indiquées par l'analyse de l'EGM à champ lointain inique la présence d'une surévaluation, y compris la comptabilisation en double d'ondes R, la surévaluation d'ondes T, le mauvais fonctionnement ou l'échec d'une électrode, des faibles connexions d'électrode, le bruit associé à des interférences électromagnétiques, des myopotentiels non cardiaques, etc. L'identification du type de surévaluation peut être déterminée par analyse du nombre et du modèle des ondes R détectées par un canal marqueur en fonction de la synchronisation des ondes R détectées à partir de l'EGM à champ lointain.
