A catheter for identifying and providing access through a tissue structure such as the fossa ovalis (72). The catheter (20) comprises an elongate flexible tubular body (26) having a proximal (22) and a distal (24) end and a retractable piercing structure (42) carried by the distal end. A detector (37) is associated with the catheter such that placement of the distal end of the catheter against the fossa ovalis can be detected.
