According to the present state of the art, endoscopic cannulation of the common bile duct and papillotomy and/or sphincterotomy of the Papilla of Vater and/or the Sphincter of Oddi is accomplished by advancing a sphincterotome (or papillotome or cannulotome) into an endoscope/duodenoscope sothat the distal tip of the sphincterotome exits the endoscope adjacent the sphincter muscles at the Papilla of Vater. The endoscope mechanisms are then manipulated to orient the distal tip of the sphincterotome to the desired position for proper cannulation of the duct. Accurate and consistent control of the length of the exposed blade is made difficult due to a number of factors. These factors include: 1) differences in the inside diameters of the outer tube and the needle knife wire, 2) the orientation of the needle knife wire within the outer tube, 3)the mismatch of tolerance of the needle knife wire and the inside diameter of the extrusion, 4)anatomy, and 5)endoscope manipulation. A sphincterotome incorporating the present invention will provide the user with an indication of the exposed blade length and will allow the physician to control the length of the exposed blade. According to one embodiment of the present invention, various visual indications are presented to the user as the needle knife is advanced from its outer sheath. These visual indications, combined with a mechanical method to hold the knife in position during catheter placement allows the use to perform precise incisions. Presently available products that may be modified according to the present invention include, but are not limited to, Boston Scientific Sphincterotomes and Needle Knives.
Dans l'état actuel de la technique, on effectue la canulation endoscopique de la voie biliaire principale et la papillotomie et/ou la sphinctérotomie de la papille de Vater et/ou du sphincter d'Oddi en faisant avancer un sphinctérotome (ou un papillotome ou un cannulatome) dans un endoscope/duodénoscope, de manière à faire sortir l'extrémité distale du sphinctérotome de l'endoscope à proximité immédiate des muscles sphincter au niveau de la papille de Vater. On manipule ensuite les mécanismes de l'endoscope afin d'orienter l'extrémité distale du sphinctérotome dans la position désirée de manière à permettre une canulation adéquate de la voie biliaire. Le réglage précis et stable de la longueur de la lame exposée est rendu difficile par un certain nombre de facteurs. Ces facteurs comprennent : 1) les différences de diamètre interne entre le tube externe et le fil du bistouri-aiguille, 2) l'orientation du fil du bistouri-aiguille à l'intérieur du tube externe, 3) l'incompatibilité de tolérance entre le fil du bistouri-aiguille et le diamètre interne de l'extrusion, 4) l'anatomie, et 5) la manipulation de l'endoscope. Un sphinctérotome équipé du dispositif décrit permet de fournir à l'utilisateur une indication de la longueur de lame exposée et permet au médecin de régler la longueur de lame exposée. Dans une forme de réalisation, diverses indications visuelles sont présentées à l'utilisateur à mesure que le bistouri-aiguille est avancé hors de sa gaine externe. Ces indications visuelles combinées à un procédé mécanique assurant le maintien de la position du bistouri pendant la mise en place du cathéter permettent à l'utilisateur d'effectuer des incisions précises.
