A surgical instrument for treating a blood vessel wall following percutaneous trans-luminal coronary angioplasty (PTCA) includes an optical system (48, 50, 52) for transmitting radiant energy, preferably UV light energy, from a laser source to the distal end portion of a PTCA catheter (30), and causing the radiant energy to exit the catheter in a relatively narrow radial band. A microprocessor controlled stepping motor is used to longitudinally reposition the band along the length of the balloon (18) of the PTCA catheter to thereby expose endothelial tissue spanned by the balloon to radiation for the purpose of inhibiting smooth muscle proliferation reducing incidence of restenosis.

L'invention concerne un instrument chirurgical qui permet de traiter la paroi des vaisseaux sanguins après une angioplastie coronarienne transluminale percutanée. L'instrument comprend un système optique (48, 50, 52) qui transmet l'énergie rayonnante, de préférence celle d'une lumière ultraviolette, entre une source laser et l'extrémité distale d'un cathéter (30) utilisée dans l'angioplastie sus-mentionnée, et pour diffuser cette énergie à la sortie du cathéter à l'intérieur d'une bande radiale relativement étroite. Un moteur pas-à-pas commandé par microprocesseur est utilisé en vue de repositionner longitudinalement la bande sur la longueur du ballonnet (18) du cathéter employé dans le type d'angioplastie considéré, ce qui permet d'exposer aux rayonnements le tissu endothélial délimité par le ballonnet et, ainsi, d'inhiber la prolifération des muscles lisses pour réduire le risque de resténose.
