This invention is a method, and apparatus (100) for in vivo electroporation therapy. Using electroporation therapy (EPT) as described in the invention, tumors treated by a combination of electroporation using the apparatus (100) of the invention, and a chemotherapeutic agent caused regression of tumors in vivo. In one embodiment, the invention provides a method of EPT utilizing low voltage, and long pulse length for inducing cell death. One embodiment of the invention includes a system for clinical electroporation that includes a needle array electrode (114) having a "keying" element that determines the set point of the therapy voltage pulse, and/or selective array switching patterns. A number of electrode applicator designs permit access to, and treatment of a variety of tissue sites. Another embodiment provides a laparascopic needle applicator that is preferably combined with an endoscope for minimally invasive EPT.
L'invention concerne un procédé et un appareil (100) utiles pour une thérapie in vivo par électroporation. Au moyen d'une thérapie par électroporation (EPT) telle que décrite dans l'invention, on a pu faire régresser in vivo des tumeurs traitées à l'aide d'une électroporation effectuée au moyen de l'appareil (100), combinée à agent chimiothérapeutique. Dans un mode de réalisation, l'invention concerne un procédé d'EPT employant une faible tension et une longue durée d'impulsion en vue de provoquer la mort de cellules. Un mode de réalisation de l'invention comprend un système d'électroporation clinique qui comporte une électrode (114) à réseau d'aiguilles présentant un élément de "modulation" qui permet de déterminer le point de réglage de l'impulsion en tension thérapeutique, et/ou les motifs de commutation de réseau sélectifs. Certains modèles d'applicateurs d'électrode permettent un accès à, et un traitement de divers sites tissulaires. Un autre mode de réalisation concerne un applicateur d'aiguille laparoscopique qui est de préférence combiné avec un endoscope pour effectuer des EPT peu vulnérantes.
