A method for in vivo, ex vivo and in vitro regeneration of cartilage and collagen. in vivo, ex vivo and in vitro regeneration and de novo formation of articular cartilage and collagen by intermittently applied hydrostatic pressure. The application of external interval loading consisting of repeated periods of applied hydrostatic pressure followed and interrupted by periods of recovery. The application of the intermittent hydrostatic pressure at physiological levels 5-10 MPa for an interval of 4 hours followed by a recovery period up to about 20 hours, said pressure applied to the cartilage cells in vitro, explants of cartilage ex vivo and in vivo to cartilage that remains intact within the joint space of diarthrotic joints. The interval loading results in the selective inhibition of matrix degrading enzymes, pro- inflammatory cytokines and chemokines that attract inflammatory cells into the joint cavity and in selective decrease of gene expression of growth factors that are inhibitory to type II collagen expression.

L'invention concerne une méthode in vivo, ex vivo et in vitro de réparation et de régénération du cartilage et du collagène. Cette invention concerne plus particulièrement une méthode in vivo, ex vivo et in vitro de régénération et de formation de novo du cartilage et du collagène articulaire par pression hydrostatique intermittente. L'application d'une charge externe par intervalle comprend des périodes répétées d'application de pression hydrostatique suivies et interrompues par périodes de récupération. Cette méthode comprend l'application de la pression hydrostatique intermittente à des niveaux physiologiques compris entre 5 et 10 MPa pendant un intervalle de temps de 4 heures, suivie par une période de récupération pouvant aller jusqu'à 20 heures, ladite pression étant appliquée sur les cellules cartilagineuses in vitro, les explants cartilagineux ex vivo et in vivo sur du cartilage restant intact dans l'interligne articulaire des articulation diarthrosiques. Le chargement intermittent entraîne l'inhibition sélective des enzymes de dégradation de matrice, des cytokines pro-inflammatoires et des chimiokines attirant des cellules inflammatoires dans la cavité articulaire et la réduction sélective de l'expression génique des facteurs de croissance inhibiteurs de l'expression du collagène de type II.
