The invention relates to the discovery that the administration of the combination of hirudin and Acetylsalicylic Acid (ASA) to patients not undergoing treatment with any thrombolytic agent, inhibits and/or prevents myocardial or cardiovascular events in the patient. The invention further relates to the pharmaceutical combination of a platelet inactivating drug, acetylsalicylic acid, and a direct thrombin inhibitor, hirudin, for the treatment of patients with acute myocardial infarction who are not undergoing thrombolysis. A pharmaceutical composition comprising the synergistic combination of hirudin and ASA, in the absence of any thrombolytic agent, is particularly useful in the therapeutic and prophylactic methods of the invention.
