A device (25) and method are disclosed for treating patients suffering from heart failure to increase cardiac output. The patient's vagus nerve (not shown) is electrically stimulated or modulated with a sequence of substantially equally spaced pulses by an implanted neurostimulator (25), and the frequency of the stimulating pulses is adjusted until the patient's heart rate reaches a target rate within a relatively stable target rate range below the low end of the patient's customary resting heart rate. The frequency of the stimulating pulses is maintained at the frequency that produced the stable target heart rate range, to maintain the target rate while the patient remains at rest. An activity sensor (44) associated with the implanted neurostimulator (25) detects physical activity of the patient and adjusts the frequency of the stimulating pulses accordingly, to elevate the heart rate during periods of physical activity by the patient. When the patient ceases the activity, vagal stimulation follows a prescribed fall-back path to a frequency that reproduces the target heart rate.

L'invention concerne un dispositif (25) et un procédé servant à traiter des patients atteints de défaillance cardiaque afin d'augmenter leur capacité cardiaque. On stimule ou on module le nerf vague du patient au moyen d'une séquence d'impulsions électriques espacées de façon régulière par un neurostimulateur implanté (25) et on règle la fréquence des impulsions de stimulation jusqu'à ce que le rythme cardiaque du patient atteigne un rythme ciblé dans une plage relativement stable au-dessous de la limite inférieure du rythme cardiaque habituel du patient au repos. On maintient la fréquence des impulsions de stimulation au niveau ayant servi à produire la plage de rythme cardiaque ciblée et stable, de façon à maintenir ce rythme ciblé tandis que le patient reste au repos. Un détecteur d'activité (44) associé au neurostimulateur implanté (25) détecte l'activité physique du patient et règle la fréquence des impulsions de stimulation en conséquence, afin d'élever le rythme cardiaque pendant les périodes d'activité physique du patient. Quand le patient cesse cette activité, la stimulation du nerf vague suit une trajectoire de chute déterminée jusqu'à une fréquence reproduisant le rythme cardiaque ciblé.
