Methods and apparatus for treating disc herniation provide a conformable device which assumes a first shape associated with insertion and a second shape or expanded shape to occlude the defect which typically follows partial dissection. The device may take different forms according to the invention, including patches (900) size to cover the defect or plugs (1002) adapted to fill the defect. In a preferred embodiment, however, the device is a gel or other liquid or semi-liquid which solidifies to occlude the defect from within the body of the disc itself. In another preferred embodiment, a mesh screen (202) is collapsed into an elongated form for the purposes of insertion, thereby minimizing the size of the requisite incision while avoiding delicate surrounding nerves. Such a configuration also permits the use of instrumentation to install the device, including, for example, a hollow tube (1206) or sheath adapted to hold the collapsed screen (1206), and a push rod (1204) to expel the collapsed device out of the sheath (1204) for use in occluding the disc defect. A device according to the invention may further include one or more anchors (902) to assist in permanently affixing the device with respect to the defect.
L'invention concerne des procédés et un appareil permettant de traiter les hernies discales. L'appareil se présente sous la forme d'un dispositif conformable qui adopte une première forme associée à l'insertion et une seconde forme expansée servant à occlure le défaut qui apparaît généralement suite à une dissection partielle. Selon l'invention, ce dispositif peut se présenter sous différentes formes, notamment des bandes (900) dimensionnées pour recouvrir le défaut ou des tampons (1002) servant à rembourrer le défaut. Dans un mode de réalisation préféré, le dispositif se présente sous la forme d'un gel ou d'un autre liquide ou semi-liquide qui se solidifie afin d'occlure le défaut depuis l'intérieur même du corps du disque. Dans un autre mode de réalisation préféré, un écran à maille (202) est plié de façon à prendre une forme allongée et à être inséré, minimisant ainsi la taille de l'incision requise tout en évitant les nerfs délicats entourant le disque. Une telle configuration permet également d'utiliser des instruments afin d'installer le dispositif, notamment un tube creux (1206) ou une gaine creuse servant à maintenir l'écran plié (1206) et une tige poussoir (1204) servant à pousser le dispositif plié hors de la gaine (1204) afin de l'utiliser pour réaliser l'occlusion du défaut discal. Un dispositif selon l'invention peut également comporter un ou plusieurs dispositifs d'ancrage ou contribuer à fixer à demeure le dispositif sur le défaut.
