Methods and systems for transvenously accessing the pericardial space via the vascular system and atrial wall, particularly through the superior vena cava and right atrial wall, to deliver a pharmacologic agent, particularly a NO- donor drug, to the heart are disclosed. A proximal connector of an infusion catheter is coupled to an infusion pump, and a distal catheter segment having a distal infusion catheter lumen end opening is disposed in the pericardial space. The implantable infusion pump is operable in conjunction with an implantable ischemia monitor to monitor the ischemic state and trigger delivery or regulate the periodic delivery of the pharmacologic agent to optimally treat ischemia. The patient may operate a patient activator that the patient when feeling ischemia symptoms to transmit a signal that is received by the implantable infusion pump and triggers delivery of a bolus and/or continuous infusion.
L'invention concerne des méthodes et des systèmes permettant l'accès par voie transveineuse à la cavité péricardique par le système vasculaire et la paroi atriale, en particulier à travers la veine cave supérieure et la paroi atriale droite, pour administrer un agent pharmacologique, en particulier un médicament donneur de NO, au coeur. Un connecteur proximal d'un cathéter de perfusion et couplé à une pompe de perfusion, et un segment de cathéter distal présentant une ouverture d'extrémité de lumière distale est disposé dans la cavité péricardique. La pompe de perfusion implantable est utilisée conjointement à un dispositif de surveillance d'ischémie implantable pour surveiller un état ischémique et déclencher la distribution ou réguler la distribution périodique de l'agent pharmacologique afin de traiter l'ischémie de manière optimale. Le patient peut commander un dispositif d'activation lorsqu'il ressent des symptômes d'ischémie afin de transmettre un signal, reçu par la pompe de perfusion implantable et activant la distribution d'un bolus et/ou une perfusion en continu.
