Methods, computer program products and apparatus determine a subject's risk of having or developing CHD using a mathematical model of in vivo concentration of LDL particles using concentration gradient factors to adjust in vitro measurements of small and large LDL particles to reflect predicted internal concentrations to thereby provide an improved model of CHD risk.
