A mobile bearing knee prosthesis enables a surgeon to convert a mobile bearing insert having articular surfaces, supported by a tibial base plate or tray from a rotating and translating prosthesis to one that rotates only. This conversion is accomplished with a fastener or locking member that connects through an opening in the insert to the tibial base plate. This prosthesis can be used as part of a total knee surgery when the surgeon chosses to use a prosthesis that incorporates a movable articular surface. In one embodiment, a projecting portion extends proximally from the insert and cooperates with a cam on the femoral component. The projecting portion can be a post extending up from the proximal surface of the insert and the femoral component includes an intercondylar surface that may contact the post to constrain the relative motion between the femoral component and the insert. In another embodiment, the insert is a two part assembly that includes a larger member with a central opening and a smaller member that fits the opening.
La présente invention concerne une prothèse articulée permettant la mise en charge grâce à laquelle un chirurgien peut transformer un insert articulé permettant la mise en charge à surfaces articulaires, soutenu par une plaque ou un plateau de base tibial et constituant une prothèse capable d'un mouvement rotation et de translation, en une prothèse capable de rotation uniquement. On accomplit cette transformation au moyen d'une attache ou d'un organe de verrouillage qui est relié à la plaque de base tibiale à travers une ouverture dans l'insert. On peut utiliser la prothèse de l'invention dans le cadre d'une prothèse totale du genou lorsque le chirurgien choisit d'utiliser une prothèse dans laquelle est intégrée une surface articulaire mobile. Dans un mode de réalisation, une partie saillante s'étend proximale à partir de l'insert et coopère avec une came sur le composant fémoral. La partie saillante peut être constituée par une tige qui s'étend vers le haut depuis la surface proximale de l'insert et le composant fémoral comprend une surface intercondylaire qui peut entrer en contact avec la tige pour limiter le mouvement relatif entre le composant fémoral et l'insert. Dans un autre mode de réalisation, l'insert est constitué par un ensemble en deux parties qui comprend un organe plus grand muni d'une ouverture centrale et un organe plus petit qui s'emboîte dans l'ouverture.
