ABSTRACT Embodiments of a container with an insert at the top are disclosed along with apparatus and methods for molding, forming and filling the container. In one form of the invention a length of a hollow tube parison is extruded and then cut, molded, and filled while positioned between main mold halves. A stopper, nozzle, or like insert is transferred to a placement arm and, when a blowing and filling assembly has been withdrawn from the top opening of the filled container, both the blowing and filling assembly and the placement arm that is carrying the insert are moved together to position the arm and insert over the container. The arm is then moved downwardly to deposit the insert within the upwardly extending portion of the parison tube at the container top opening. Before or after the arm is withdrawn, upper sealing molds may be closed to form the upwardly extending portion of the parison around the insert to at least partially encapsulate the insert. A frangible web may also be formed by the upper sealing molds in a portion of the parison circumferentially above or directly against the insert. A break-off overcap may also be formed above the frangible web. In use, the overcap may be broken away at the frangible web to expose an upper surface of the insert and permit access to the insert.
