A system for efficient delivery of radio frequency (RF) energy to cardiac tissue with an ablation catheter used in catheter ablation, with new concepts regarding the interaction between RF energy and biological tissue. In addition, new insights into methods for coagulum reduction during RF ablation will be presented, and a quantitative model for ascertaining the propensity for coagulum formation during RF ablation will be introduced. Effective practical techniques are presented for multichannel simultaneous RF energy delivery with real-time calculation of the Coagulum Index, which estimates the probability of coagulum formation. This information is used in a feedback and control algorithm which effectively reduces the probability of coagulum formation during ablation. For each ablation channel, electrical coupling delivers an RF electrical current through an ablation electrode of the ablation catheter and a temperature sensor is positioned relative to the ablation electrode for measuring the temperature of cardiac tissue in contact with the ablation electrode. A current sensor is provided within each channel circuitry for measuring the current delivered through said electrical coupling and an information processor and RF output controller coupled to said temperature sensor and said current sensor for estimating the likelihood of coagulum formation. When this functionality is propagated simultaneously through multiple ablation channels, the resulting linear or curvilinear lesion is deeper with less gaps. Hence, the clinical result is improved due to improved lesion integrity.
L'invention concerne un système d'apport efficace d'énergie HF à un tissu cardiaque au moyen d'un cathéter d'ablation s'utilisant dans des opérations d'ablation par cathéter, qui emploie de nouveaux concepts quant à l'interaction entre l'énergie HF et le tissu biologique. De plus, l'invention présente de nouvelles connaissances relatives aux procédés de réduction de caillot pendant une ablation HF, ainsi qu'un modèle quantitatif permettant de déterminer la tendance à former un caillot pendant une ablation HF ; et des techniques pratiques efficaces utiles pour l'apport d'énergie HF simultanée multicanaux avec calcul en temps réel de l'indice de caillot, qui indique la probabilité de formation d'un caillot. Ces informations sont utilisées dans un algorithme de rétroaction et de commande qui permet de réduire efficacement la probabilité de formation d'un caillot pendant l'ablation. Pour chaque canal d'ablation, un couplage électrique fournit un courant électrique HF par une électrode d'ablation du cathéter d'ablation, et une sonde thermique est positionnée par rapport à l'électrode d'ablation pour mesurer la température du tissu cardiaque en contact avec l'électrode d'ablation. Un détecteur de courant est prévu à l'intérieur de chaque circuit de canal pour mesurer le courant fourni par ledit couplage électrique, ainsi qu'un processeur d'informations et un organe de commande de sortie HF couplés à la sonde thermique et au détecteur de courant pour estimer la probabilité de formation de caillot. Quand cette fonctionnalité est propagée simultanément dans de multiples canaux d'ablation, la lésion linéaire ou curviligne obtenue est plus profonde et présente moins de trous. De cette manière, le résultat clinique est amélioré grâce à l'amélioration de l'intégrité de la lésion.
