A multi-part form includes a printable face ply that is die cut to create a detachable wristband, labels and/or cards and is particularly useful in a hospital-patient environment. The face ply includes a first major surface and a second major surface. A pressure sensitive adhesive is located on of least a portion of one of the major surfaces. The multi-part form includes a first portion, second portion and intermediate portion. The multi-part form further includes a liner ply with first and second major surfaces where a release coating is located on at least a portion of one of major surfaces of the liner ply. The liner ply is adhered to the pressure sensitive adhesive on the face ply.
L'invention concerne une forme à plusieurs parties comprenant une couche face imprimable coupée à la forme afin de créer un bracelet, des étiquettes et/ou des cartes détachables utilisées, en particulier, dans un environnement hôpital-patient. La couche face comprend une première surface principale et une seconde surface principale. Un adhésif autocollant est placé sur l'une des surfaces principales. La forme multipartie comprend une première partie, une seconde partie et une partie intermédiaire. Elle comprend également une couche de garniture associée aux première et seconde surfaces principales, un revêtement détachable étant placé sur au moins une partie de l'une des surfaces principales de la couche de garniture. Ladite couche adhère à l'adhésif autocollant sur la couche face .
