The present invention discloses a method for diagnosing a cardiac complication in a patient due to coronary catheterization, comprising the steps of (a) obtaining at least one baseline sample from the patient during or after coronary catheterization, and then (b) obtaining at least one test sample fro m the patient after coronary catheterizat ion, (c) determining a change of the level of an ANP-Type peptide, or a variant thereof, between the baseline sample and the test sample, wherein a significant decrease in the level of the ANP-type peptide according to step c) indicates a cardiac complication. In a preferred embodiment also a BNP-type peptide is measured, wherein a significant increase of the level of the BNP-type peptide indicates a cardiac complication. ANP-type peptide and BNP-type peptide belong to the group of natriuretic peptides. Preferred peptides include ANP (atrial natriuretic peptide) and BNP (brain natriuretic peptide), and the respective N-terminal fragments thereof, NT-proANP and NT-proBNP. The invention allows early diagnosis of cardiac complications due to or caused by coronary catheterization (introduction of a catheter), e.g. angiography, angioplasty, ballo on dilatation, percutaneous coronary intervention (PCI), or stent implantation. Examples of complications which are detectable include myocardial ischemia and myocardial necrosis, even if they are asymptomatic or not clinically apparent. The invention also describes the additional measurement of markers of necrosis (e.g. troponins) and markers of inflammation (e.g. interleukins).
La présente invention concerne un procédé permettant de diagnostiquer une complication cardiaque chez un patient due à un cathétérisme coronaire, lequel procédé comprend les étapes consistant à (a) obtenir au moins un échantillon de ligne isoélectrique provenant du patient pendant ou après un cathétérisme coronaire, puis (b) obtenir au moins un échantillon pour essai provenant du patient après le cathétérisme coronaire, (c) déterminer le changement de niveau d'un peptide de type ANP, ou une variante de celui-ci, entre l'échantillon de ligne isoélectrique et l'échantillon pour essai, une diminution significative du niveau du peptide de type ANP selon l'étape c) indiquant une complication cardiaque. Selon un mode de réalisation préféré, un peptide de type BNP est également mesuré, une augmentation significative du niveau du peptide de type BNP indiquant une complication cardiaque. Le peptide de type ANP et le peptide de type BNP appartiennent au groupe des peptides natriurétiques. Des peptides préférés incluent l'ANP (peptide natriurétique auriculaire) et le BNP (peptide natriurétique de type B), ainsi que les fragments N-terminal respectifs de ceux-ci, le NT-proANP et le NT-proBNP. L'invention permet un diagnostic précoce des complications cardiaques dues à un cathétérisme coronaire (introduction d'un cathéter) ou causées par celui-ci, par exemple une angiographie, une angioplastie, une dilatation au ballonnet, une intervention coronarienne percutanée (ICP) ou une implantation d'endoprothèse vasculaire. Des exemples de complications qui peuvent être détectées incluent l'ischémie myocardique et la nécrose myocardique, même si elles sont asymptomatiques ou non apparentes cliniquement.
