ABSTRACT OF THE DISCLOSURE: A non-reproducible prescription slip comprises a first surface portion on which a name of a prescribing physician is printed, a second surface portion on which identification indicators for a recipient of the prescription is printed, a third surface portion on which a signature indicator for a signature of the physician is printed, and a fourth surface portion on which a security ink pattern is printed, the prescription to be written on the pattern by the physician. A pad of the above described prescription slips is also disclosed, the pad comprising: a stack of said prescription slips and advertizement sheets, said advertizement sheets being interlaced with said prescription slips at a given interval; and a binding for holding said stack together, the binding having means for attaching the prescription slips and advertizement sheets in a detachable fashion for easy removal from the pad. Patient safety and the physician's ease of mind are improved.
