A retracting syringe of one use has a plunger comprising a handle portion and a needle retention portion forward of the handle portion which serves as the plunger and preferably also serves to trigger the retraction mechanism when the handle is moved forward beyond the forward end-of-injection position. The positive locking stricture in the syringe barrel permits one withdrawal of the handle in the rearward direction to fill the variable fluid chamber and one movement in the forward direction to empty the variable fluid chamber but thereafter limits the rearward movement of the needle retention chamber and/or the handle to ensure that the syringe cannot be reused even if the user fails to trigger the retraction device to retract the needle. In one embodiment, the positive locking structure is a stop in the syringe barrel which allows a diametrically enlarged portion of the needle retention chamber to move past the lock when going forward and having a two part separable handle. The handle portion merely separates after one use and is removed from the syringe without withdrawing the needle retention chamber or needle which are protected by the barrel. In a second embodiment, the handle behind the needle retention chamber has a plurality of stepped serrations and a clip which is movable with respect to both the handle and the barrel. After one use of the syringe, points on the clip dig into the inner surface of the barrel to prevent rearward movement. In a third embodiment the positive locking structure is a clip fixed in an enlarged back opening of the barrel with springing points which are protected from a plurality of stepped serrations on the handle by sliding collar which can move only one way relative the handle like those of the second embodiment. After one use the sliding collar slides forward with the handle allowing the handle to be fully depressed to make the injection. Any attempt to withdraw the handle after one injection is prevented by the points digging into the serrations on the handle. Since the handle cannot be withdrawn a second time, the syringe cannot be refilled. The one use feature disclosed herein can be applied to a nonretracting syringe to prevent reuse.
