A pacemaker (100) is disclosed that includes circuitry (175) for monitoring the current and/or voltage delivered to the heart during a pacing pulse. A microprocessor (225) connects to the monitoring circuitry and analyzes the current (and/or voltage) to determine when capture occurs. When capture occurs, the microprocessor terminates the pacing pulse to save energy. Conversely, if capture does not occur within a predetermined maximum time period, or if the pacing pulse amplitude falls below a predetermined threshold value, the processor immediately causes a safety pulse to fire to insure a regular beating of the heart. By monitoring the current and/or voltage delivered, and by permitting the pacing pulse width to be variable, the pacemaker of the present invention can closely track the pacing threshold of the patient with a minimum expenditure of energy.

La présente invention concerne un stimulateur cardiaque (100) équipé d'un circuit (175) surveillant l'intensité et/ou la tension fournie au coeur pendant l'impulsion de stimulation. En l'occurrence, un microprocesseur (225) connecté au circuit de surveillance analyse l'intensité (et/ou la tension) afin de déterminer le moment où se produit la capture. Lorsque la capture par le coeur se produit, le microprocesseur met fin à la stimulation pour économiser de l'énergie. Inversement, si la capture ne se produit pas pendant un créneau défini, ou si l'amplitude de l'impulsion de stimulation descend en dessous d'une valeur de seuil définie, le processeur provoque immédiatement le départ d'une impulsion de sécurité destinée à garantir un battement régulier du coeur. Grâce à la surveillance de l'intensité et/ou de la tension fournie, et parce que l'on admet ici que la longueur de l'impulsion de stimulation soit variable, le stimulateur cardiaque de la présente invention est capable de s'adapter au plus près du seuil de stimulation du patient, et ce, avec un minimum de dépense d'énergie.
