A pharmaceutical composition for oral administration comprising PTH, wherein the in vitro release of PTH-when tested in a dissolution test of pharmacopoeia standard-is delayed with at least 2 hours and once the release starts, at least 90% w/w such as, e.g., at least 95% or at least 99% of all PTH contained in the composition is released within at the most 2 hours. The composition may also comprises a calcium containing compound and/or a vitamin, D. In particular, PTH is administered in combination with a calcium-containing compound for the treatment or prevention of bone-related diseases, so that I) an effective amount of a calcium-containing compound is administered to lower the plasma level of endogenous PTH, and II) an effective amount of PTH is administered to obtain a peak concentration of PTH once the endogeneous PTH level is lowered. This present a potential therapeutic or prophylactic regimen for bone-related disorders including osteoporosis.
L'invention concerne une composition pharmaceutique destinée à être administrée par voie orale, contenant une PTH, et dans laquelle la libération in vitro de la PTH lorsqu'elle est effectuée dans un test de dissolution de pharmacopée classique est retardée d'au moins 2 heures, et dans laquelle, lorsque la libération commence, au moins 90 % en poids, par exemple au moins 95 % ou au moins 99 %, de toute la PTH contenue dans la composition est libéré en 2 heures tout au plus. La composition peut également contenir un composé de calcium et/ou une vitamine D. La PTH est administrée, en particulier, en combinaison avec un composé contenant du calcium dans le traitement ou la prévention des maladies osseuses, de manière I) qu'une quantité efficace d'un composé contenant du calcium soit administrée afin de réduire le niveau de plasma de la PTH endogène, et II) qu'une quantité efficace de PTH soit administrée afin d'obtenir une concentration maximale de PTH une fois que le niveau de PTH endogène est réduit. L'invention propose un schéma posologique thérapeutique ou prophylactique potentiel utilisé dans les maladies osseuses, y compris l'ostéoporose.
