Methods are provided for partial aortic occlusion for cerebral perfusion augmentation in patients suffering from global or focal cerebral ischemia. Alternatively, the methods can be used to partially occlude aortic blood flow to condition the spinal cord to secrete neuro-protective agents prior to abdominal aortic aneurysm repair. Partial occlusion of a vessel can be accomplished by a device comprising an elongate catheter (102), a distally mounted expandable occludent (104), a variable flow mechanism, and a manometer (112). The occludent (104) may comprise two conical shells (118, 136) or an inflatable ring-shaped balloon (130) disposed about a cylindrical sleeve. Other medical devices, such as an atherectomy catheter, can be inserted distal the occludent to provide therapeutic intervention.
L'invention concerne des procédés d'occlusion aortique partielle permettant d'augmenter l'irrigation sanguine cérébrale chez des patients souffrant d'ischémie cérébrale globale ou focale. Dans une variante, ces procédés peuvent être utilisés pour occlude partiellement un écoulement de sang aortique afin d'amener la moelle épinière à sécréter des agents neuroprotecteurs avant toute réfection d'un anévrisme aortique abdominal. On peut procéder à l'occlusion partielle d'un vaisseau à l'aide d'un dispositif comprenant un cathéter allongé (102), un dispositif d'occlusion extensible (104) monté de manière distale, un mécanisme à flux variable, et un manomètre (112). Le dispositif d'occlusion (104) peut être muni de deux enveloppes coniques (118, 136) ou d'un ballonnet gonflable de forme annulaire (130), placé autour d'un manchon cylindrique. D'autres dispositifs médicaux, comme un cathéter d'athérectomie, peuvent par ailleurs être insérés de manière distale par rapport au dispositif d'occlusion, afin de permettre une intervention chirurgicale.
