A system for providing feedback regarding a patient's condition is disclosed. In an embodiment, a plastic container of disposable wipes includes a first label mounted on the container and occluding an entry into the container. A second label is also mounted on the container via attachment to the first label. The second label includes a template that is mounted face down. In operation, the second label is removed and placed on the patient's medical chart and observations regarding the patient's skin condition are marked on the template provided on the second label.
