Partial or total occlusions of fluid passages within the human body are removed by positioning an array of optical fibers in the passage and directing treatment radiation pulses along the fibers, one at a time, to generate a shock wave and hydrodynamic flows that strike and emulsify the occlusions. A preferred application is the removal of blood clots (thrombi and emboli) from small cerebral vessels to reverse the effects of an ischemic stroke. The operating parameters and techniques are chosen to minimize the amount of heating of the fragile cerebral vessel walls occurring during this photoacoustic treatment. One such technique is the optical monitoring of the existence of hydrodynamic flow generating vapor bubbles when they are expected to occur and stopping the heat generating pulses propagated along an optical fiber that is not generating such bubbles.
Le procédé de la présente invention consiste, pour éliminer du le corps humain des occlusions partielles ou totales de passages de fluides, à mettre en place dans le passage considéré une matrice de fibres optiques et à envoyer par les fibres, une fibre à la fois, des impulsions de rayonnement de traitement, de façon à produire une onde de choc et un flux hydrodynamique qui percute et émulsionne les occlusions. L'un des domaines d'application préféré est l'élimination des caillots de sang (thrombus et emboles) dans les petits vaisseaux cérébraux pour inverser les effets d'une attaque ischémique. Les techniques et les paramètres de mise en oeuvre sont sélectionnés de façon à minimiser le chauffage des fragiles parois des vaisseaux cérébraux intervenant lors de ce traitement photoacoustique. L'une des techniques mises en oeuvre dans ce cadre consiste en une surveillance optique de la présence d'un flux hydrodynamique générant des bulles de vapeur lorsqu'elles sont susceptibles de se produire, et l'arrêt des impulsions thermogénératrices se propageant dans une fibre optique ne dégageant pas de telles bulles.
