ABSTRACT OF THE DISCLOSURE A one piece plug member can be mounted on a cross sectional irregularity of a generally rectangular concrete form tie rod in order to produce an outer cavity in a mass of concrete poured in the form. The plug member comprises a body made of plastic material, defining a frusto-conical outer surface, and formed with an axial and cylindrical hole defining a constriction. The constriction is constituted by an annular wall formed in the hole at the end of larger diameter of the body. A method is also provided to mount the plug member on the tie rod. In accordance with this method, the plastic body is heated to increase its malleability and allow it to deform, and the tie rod is slid into the body's hole until the constriction reaches the cross sectional irregularity where the body cools and shrinks onto the tie rod.
