The present invention is directed to a polyaxial bone anchor for attaching a rod to a bone comprising an anchor member for attachment to the bone, a body member having a U-shaped channel for receiving the rod and a compressible recess for receiving a head of the anchor member such that the anchor member can initially polyaxially angulate with respect to the body member, a collar slidably disposed about the body member and capable of compressing the recess around the head, and a fastener capable of pressing the rod against the collar. The body member may define a first axis, an upper bounding edge, and a lower bounding edge, and the lower bounding edge may include a countersunk region to permit increased angulation of the anchor member with respect to the first axis when the anchor member is oriented toward the countersunk region. Other structures for providing increased angulation of the anchor member are disclosed as well. Further, the present invention is directed to methods of fixation of the cervical region of the spine.
L'invention concerne un élément d'ancrage osseux polyaxial permettant de fixer une tige sur l'os, une partie corps présentant un canal en U destiné à recevoir la tige, et un évidement compressible destiné à recevoir la tête d'un élément d'ancrage, de manière à autoriser une angulation polyaxiale initiale de l'élément d'ancrage relativement à la partie corps, un manchon monté de manière coulissante autour de la partie corps et permettant de comprimer l'évidement autour de la tête, et un élément de fixation permettant de presser la tige contre le manchon. La partie corps peut définir un premier axe, un bord de délimitation supérieur, et un bord de délimitation inférieur, et le bord de d'élimination inférieur peut comporter une zone fraisée afin de permettre une angulation accrue de l'élément d'ancrage par rapport au premier axe lorsque l'élément d'ancrage est orienté en direction de la zone fraisée. D'autres structures permettant une angulation accrue de l'élément d'ancrage sont également décrites. L'invention concerne en outre des méthodes de fixation de la région cervicale de la colonne vertébrale.
