A proscription pad comprises a plurality of units each comprising an associated preprinted prescription leaf and preprinted cheque leaf; the preprinted prescription leaf bears a preprinted prescription for a distinct pharmaceutical product as well as a zone for entry of patient information and a zone for entry of the signature of the prescribing physician; the cheque leaf bears on one face a preprinted cheque in favour of a dispensing pharmacist, and has a value based on the value of the prescribed product and a dispensing fee, and has an endorsing zone preprinted with a dispensing acknowledgement legend relating to the preprinted prescription with an entry portion for entry of the endorsing signature of the dispensing pharmacist; the cheque leaf is preferably coded to identify the physician. The pad can be employed for the prescribing of free starter dosages of the pharmaceutical product for the patient or to provide the patient with a discount; the pharmacist is reimbursed simply by depositing the endorsed preprinted cheque in his bank account, and the control body on whose account the cheque is drawn is able to monitor the use of the preprinted prescriptions by physicians and provide the pharmaceutical company with valuable information.
L'invention porte sur un carnet d'ordonnances composé d'une série d'unités comprenant chacune un feuillet ordonnance et un feuillet chèque préimprimés. Le feuillet ordonnance préimprimé comporte une ordonnance préimprimée pour un produit pharmaceutique distinct ainsi qu'une zone pour les données sur le patient et une zone pour la signature du prescripteur. Le feuillet chèque comporte, sur un côté, un chèque préimprimé au nom d'un pharmacien diplômé, affiche une valeur fondée sur la valeur du produit prescrit et sur les frais d'exécution d'ordonnances et comprend une zone d'endossement sur laquelle est préimprimée une légende de reconnaissance de l'ordonnance se rapportant à l'ordonnance préimprimée et qui inclut une partie pour la signature d'endossement du pharmacien diplômé. De préférence, le feuillet chèque est codé de façon à identifier le médecin. Le carnet peut être utilisé pour les ordonnances de doses de départ gratuites du produit pharmaceutique données au patient ou pour donner un rabais à ce dernier. Le pharmacien est remboursé simplement en déposant le chèque préimprimé endossé dans son compte bancaire. L'organisation de contrôle, dans le compte de laquelle le chèque est tiré, est ainsi capable de contrôler l'utilisation par les médecins des ordonnances préimprimées et de transmettre de précieux renseignements à l'entreprise pharmaceutique.
