ABSTRACT BUSINESS FORM The invention concerns business forms and more particularly such forms designed to provide from a single sheet a combined delivery receipt form/note for a supplier and a sealed document for a customer listing items delivered and an invoice therefor. The business form comprises a sheet of paper (12) divided into three substantially equal panels (17, 18, 19) by first and second fold lines (21, 22) extending between the top and bottom edges. The fold line (21) is formed with perforations to aid easy detachment of the left hand panel (17) which forms a delivery receipt note and which bears transfer material in the form of a carbon patch (26) on its inner surface. The panels (18, 19) carrying adhesive (23) around their free edges and are designed to be printed with details of items being delivered and an invoice therefor and then to be brought together and sealed around their edges to provide a sealed invoice. The customer is intended to sign the outside of the delivery note in an area (32) corresponding to the transfer material so that the signature is transferred through to the sealed document. The supplier's representative then detaches the receipted panel (17) and the customer passes the sealed document to his accounts department.
