ID-89-12 ABSTRACT OF THE DISCLOSURE An inexpensive business form construction including a sheet (48) of face stock having one surface (50) adapted to receive printed information and an opposed surface (52) provided with pressure sensitive adhesive (54), a sheet (40) of release liner removably adhered to the opposed surface (52) by the pressure sensitive adhesive (54), a closed die cut (52) in the face stock sheet to define a removable panel (28) that has pressure sensitive adhesive (54) on one side (52) thereof and which may be pulled from the release liner (40), and a closed die cut (72) in the release liner sheet (40) and located inwardly of at least two opposed edges of the face stock sheet (48) and outwardly of at least two opposed sides of the die cut (52) in the face stock sheet (48).
