The present invention relates to a prostheses and instruments for joint repair and replacement. In a transosseous core approach of the present invention, the joint is entered through a pathway provided in a portion of a joint bone. Such pathway is preferably made by taking out a bone core from the bone in or adjacent to the joint, wherever possible without substantially compromising physical integrity and physiological viability of the joint. Typically the main route for the transosseous core with a less-accessible bone of the joint in order to facilitate treatment of articular surfaces and/or other structures in the joint. The transosseous pathway into the joint is preferably provided by novel surgical instruments such as a core cutter (30), cartilage punch (40), hemostatic device (60) and retractable axial/transaxial cutting device (33). Implants modules are also provided which can be inserted into the joint through the transosseous pathway and assembled in situ inside the joint to form an implant assembly.
La présente invention concerne des prothèses et des instruments permettant le remplacement et la réparation d'articulations. Dans une approche de cylindre transosseux de cette invention, on accède à l'articulation à travers un voie d'accès qui est ménagée dans une partie d'un os de l'articulation. Cette voie d'accès est de préférence ménagée en retirant un cylindre osseux de l'os qui fait partie de l'articulation ou qui est adjacent à l'articulation, à n'importe quel endroit possible, sans compromettre sensiblement l'intégrité physique et la viabilité physiologique de l'articulation. Généralement, la voie principale pour l'approche de cylindre transosseux traverse un os plus accessible de l'articulation, qui peut être aligné avec un os moins accessible de l'articulation, afin de faciliter le traitement de surfaces articulaires et/ou d'autres structures dans l'articulation. La voie d'accès transosseuse dans l'articulation est de préférence pratiquée au moyen de nouveaux instruments chirurgicaux, tels qu'un dispositif de prélèvement de carotte, un emporte-pièce de cartilage, un dispositif hémostatique et un dispositif de découpe axiale/transaxiale rétractable. La présente invention concerne également des modules d'implant qui peuvent être insérés dans l'articulation à travers la voie d'accès transosseuse et assemblés in situ à l'intérieur de l'articulation, afin de constituer un ensemble implant.
