A prosthetic joint system is provided for use when replacement of bone is required. The system includes (a) an elongated stem (13) having a joint motion surface (24) at one end and an outside surface which includes at least one tap- ered portion (26); (b) a sleeve (32) for fixation to the patient's bone which has an internal taper (50) for locking engagement with the at least one tapered portion (26); (c) a bone replacement element (22, 54, 64), which can be a bone graft (22); and (d) means (26, 28, 30, 34, 40, 42, 52, 56, 66, 68) for attaching the bone re- placement element (22, 54, 64) to the stem (13) at a location between the joint motion surface (24) and the sleeve (32). In the case of bone grafts (22), the sys- tem can include a second sleeve (34) for implantation in the graft (22) and a sec- ond tapered portion (28) on the stem (13) for locking engagement with the sec- ond sleeve (34). Preferably, a series of mating stems (13), sleeves (32), and bone replacement elements (54, 64) (or second sleeves (34) in the case of bone grafts (22)) are provided.
