A ram block guide system for a ram-type blow out preventer, includes a blow out preventer main body having a horizontal cavity adapted to receive a ram block. An access opening is provided at a remote end of the horizontal cavity. A ram block is provided having an axis and an opening parallel to the axis. A closure door is provided which is adapted to close the access opening. A ram shaft extends through the closure door and is secured to the ram block along the axis. A guide member is secured to the closure door. The guide member extends into the horizontal cavity when the closure door is placed in a closed position closing the access opening. The guide member extends into the opening in the ram block, to provide support for the ram block and ensure proper alignment with the access opening.
