The elongated structure has an interior surface (106) and an exterior surface (108), a first opening (110) at a proximate end (112), and a second opening (116) along a length of the elongated structure. The sleeve has an interior surface (202) and an exterior surface (204) with a protrusion (206). The protrusion is adapted to extend through the second opening to the exterior surface of the elongated structure forming a gripping member.
