The invention concerns a system for monitoring patients at home comprising interface means in the patient's home, interface means at the doctor's, a server connected by a network such as a telephone network to the interface means in the patient's home and the interface means at the doctor's, said server receiving from the interface in the patient's home values of physiological parameters measured by the patient or by sensors and comprising means for storing a medical record, corresponding to a certain time interval, concerning the values of the physiological parameters. Said system further comprises processing means which determine on the basis of the patient's medical record for at least one of the physiological parameters, modelling probabilities of the patient being in various possible physiological conditions, and said processing means, calculate, on the basis of said modelling operations and the latest value transmitted to the server for said parameter, the probability of the patient being in said physiological conditions, and this is repeated for each of the various conditions.

Système de suivi de patients à domicile comportant des moyens d'interface chez le patient, des moyens d'interface chez un médecin, un serveur relié par un réseau tel qu'un réseau téléphonique aux moyens d'interface chez le patient et aux moyens d'interface chez le médecin, ledit serveur recevant des moyens d'interface chez le patient des valeurs de paramètres physiologiques mesurées par le patient ou par des capteurs et comportant des moyens pour mémoriser un historique, correspondant à une certaine période de temps, sur les valeurs de paramètres physiologiques. Le dit serveur comportant en outre des moyens de traitement qui déterminent en fonc-tion de l'historique du patient pour au moins l'un des paramètres physiologques, des modélisations des probabilités de se trouver dans différents états physiologiques possibles pour le patient, et en ce que lesdits moyens de traitement calculent, en fonction de ces modélisations et de la dernière valeur transmise au serveur pour ledit paramètre, la probalité de se trouver dans ces états physi-ologiques, et ce pour chacun de ces différents états.
