ABSTRACT OF THE DISCLOSURE For binding several continuous superimposed forms stitches are applied to each of the perforated margins of the forms, which stitches are derived from a continuous tape of width equal to the width of the stitches. The stitches are applied by means of a pair of opposing cylindrical heads rotating vertically and symmetrically relative to the forms, with motion synchronized between them and in respect to the motion of the rolls driving the forms, and in cooperation with a tape-halting lever and a tape-unreeling lever. One head is provided with a press and a punch and the other head is provided with a press and a notch which, during each revolution, are juxta- posed to the press and the punch of the first head, respectively. During each revolution of the heads a length of adhesive tape is caught beneath the press of the first head and secured to the form moving between the heads is then incised by the punch of the first head, and finally is torn away, producing a bind m g stitch, thereafter, a tooth of a drive roll causes the leading part of the stitch to pass into one of the marginal perforations of the form, and a roll beneath the form folds the stitch back, securing it to the bottom page of the form.
