A safety penetrating instrument for establishing a portal in an anatomical cavity for performing endoscopic procedures includes a penetrating member (32) received within a portal sleeve (21) for introducing the portal sleeve through a cavity wall into the anatomical cavity, a distally biased safety member (56) having a distal end movable between an extended position protesting a sharp tip (38) of the penetrating member (32) and a retracted position exposing the sharp tip (38), a handle (22) for manually moving the safety member (56) to the retracted position and a locking and releasing mechanism (86) for locking the safety member (56) in the retracted position and releasing the safety member (56) to the extended position in response to distal movement of an operating member (164) upon penetration of the safety member instrument into the anatomical cavity, the operating member (164) being preferably carried by the penetrating member.
