A pharmaceutical composition for application to an area of skin of a subject for local and/or systemic treatment of a COX-2 mediated disorder comprises a backing sheet that is flexibly conformable to the area of skin, the backing sheet having opposing surfaces that are respectively distal and proximal to the skin when applied; and a coating on the proximal surface of the backing sheet that comprises (a) an adhesive, (b) an active agent comprising a selective COX-2 inhibitory sulfonamide drug of low water solubility, and (c) a solvent system for the active agent, wherein the active agent is in a therapeutically effective total amount and the solvent system is selected with regard to composition and amount thereof to be effective to maintain the active agent substantially completely in solubilized form. A method of local treatment of a site of pain and/or inflammation in a subject comprises applying the composition to a skin surface of the subject, preferably at a locus overlying or adjacent to the site of pain and/or inflammation, and leaving the composition in place for a time period effective to permit delivery of a locally therapeutic amount of the active agent. A method of systemic treatment of a subject having a COX-2 mediated disorder comprises applying the composition to a skin surface of the subject, and leaving the composition in place for a time period effective to permit transdermal delivery of a therapeutic amount of the active agent.
L'invention concerne une composition pharmaceutique destinée à être appliquée sur une zone de la peau d'un sujet pour traiter chez celui-ci, de façon locale et/ou systémique, un trouble médié par la COX-2. Cette composition comprend une feuille dorsale qui peut épouser par souplesse la zone de la peau ou est appliquée la composition et qui comporte deux surfaces opposées, respectivement distale et proximale par rapport à la peau sur laquelle la composition est appliquée , ainsi qu'un revêtement situé sur la surface proximale de la feuille dorsale, lequel comprend : (a) un adhésif ; (b) un principe actif comprenant un sulfonamide inhibant de façon sélective la COX-2, de faible hydrosolubilité ; et (c) un système solvant pour le principe actif. Ledit principe actif est présent en une quantité totale thérapeutiquement efficace et ledit système solvant est sélectionné, en ce qui concerne sa composition et sa quantité, de façon à être efficace pour maintenir le principe actif sensiblement complètement sous forme solubilisée. L'invention concerne aussi un procédé de traitement local d'un site de douleur et/ou d'inflammation chez un sujet, lequel consiste à appliquer ladite composition sur une surface de la peau dudit sujet, de préférence en un emplacement recouvrant ledit site ou adjacent à celui-ci, et à laisser ladite composition en place pendant une période permettant l'administration d'une quantité localement thérapeutique du principe actif. L'invention concerne en outre un procédé de traitement systémique d'un sujet présentant un trouble médié par la COX-2, lequel consiste à appliquer la composition sur une surface de la peau d'un sujet et à laisser cette composition en place pendant une période permettant l'administration transdermique d'une quantité thérapeutique du principe actif.
