ABSTRACT A shipping form unit including an envelope made of a pro- tective material, one panel of which is provided with an adhesive material for attaching the same to a package, and the other panel being of a transparent material. A first set of form sheets is located within, and detachably engaged with, the envelope, and a second set of form sheets is superposed on the envelope, and detachably engaged with the envelope and the first set of form sheets. The sheets of the first and second sets of form sheets are provided with copy reproducing means so that indicia written on the top sheet is transferred to all copies of both sets of form sheets.
