A surgical access device (34) includes a single valve (35) that forms a seal with the body wall (21) and provides an access channel (32) into a body cavity (18). The valve (35) has properties for creating a zero seal in the absence of an instrument, an instrument seal in the presence of instruments, or a hand port to provide access into the cavity (18). The valve can include a gel, preferably an ultragel comprised of an elastomer and an oil providing elongation greater than 1000 percent and durometer less than 5 Shore A. A method for making the surgical access device includes the combining of a gelling agent with an oil, preferably by molding. A method for use includes steps for creating an opening with the instrument. The valve and method are particularly adapted for laparoscopic surgery wherein the abdominal cavity is insufflated with a gas.
L'invention concerne un dispositif d'accès chirurgical comprenant une valve unique formant un joint d'étanchéité avec la paroi du corps et constituant un canal d'accès à une cavité corporelle. La valve possède des propriétés permettant de créer un joint zéro en l'absence d'un instrument ainsi qu'un joint d'instrument avec des instruments présentant une large gamme de diamètres. La valve peut comprendre un gel et, de préférence, un ultragel comprenant un élastomère et une huile générant un allongement supérieur à 1000 pour cent et une dureté inférieure à 5 sur l'échelle Shore A. Ladite valve peut être utilisée comme une ouverture manuelle, l'instrument pouvant être le bras du chirurgien, donnant ainsi accès à la cavité. L'invention concerne également un procédé de fabrication du dispositif d'accès chirurgical, consistant à combiner un agent gélifiant avec une huile, de préférence au cours d'un procédé de moulage. L'invention concerne également un procédé d'utilisation du dispositif consistant à créer une ouverture avec l'instrument. Dans un procédé particulier, un organe peut être retiré de la cavité corporelle par l'intermédiaire de la valve de manière à créer un joint d'organe pendant que l'organe est à l'extérieur de la cavité corporelle. Cette valve et ce procédé sont particulièrement adaptés pour la chirurgie laparoscopique au cours de laquelle un gaz est insufflé dans la cavité abdominale, nécessitant ainsi le joint zéro, le joint d'instrument, et le joint d'organe dans diverses interventions.
