The present invention provides a system and method for creating, maintaining, and analyzing incident reports as part of an incident report (500) database from anywhere in the world. Embodiments of the present invention include an incident report database (40) accessible through a universal client, i.e. web- browser functionality, which provides login capabilities, report creation and analysis, and administration of reports and user functionality. PDA (50) report creation, uploading and template downloading capabilities are also provided. Users may be categorized into different permission levels providing varying degrees of accessibility to the database. Reporting options, also depending on a user's permission level, include: creating a report, searching for a report, listing all reports, creating a text report, and creating a graphical report.
