ABSTRACT OF THE DISCLOSURE A tag has first, second and third planar segments, each segment having opposite, front and back planar surfaces bounded by at least one edge extending between the planar surfaces. Two, flexible, segment connectors respectively connect portions of the edges of the first and second and second and third segments in a manner such that at least the front surfaces of the segments are in a common plane when the segment connectors are not flexed. The front surfaces of the segments can be printed, therefore, in a one-sided printing operation. When the segment connectors are flexed by folding, the front surfaces of the first and second segments are juxtaposed and one of the surfaces of the third segment is juxtaposed with the back surface of one of the first and second segments. This covers the printing on the front surfaces of the first and second segments, until the segment connector therebetween is unfolded to expose it. The tag is attached to an item, by adhesive on the back surface of one of the segments, or by a strap forming one of the segment connectors, or by a tail projecting form one of the segments.
