A toothbrush is provided having a hollow handle forming a toothpaste receiving reservoir. A head having bristles for brushing the teeth of a user extends from the handle and has an internal passageway for transporting toothpaste between the reservoir and the bristles. A flexible one-way valve formed by two opposed, abutting thin walled flaps is located at the bristle end of the internal passageway for controlling dispensing of toothpaste onto the bristles. A piston mechanism located in the reservoir squeezes toothpaste from the reservoir into the passageway, the piston mechanism being activated by a cap attached to one end of the handle. The cap is used to drive the piston mechanism to compress the toothpaste within the reservoir and the passageway, which compression activates the one-way valve to expel toothpaste onto the bristles.
