A transdermal extended-delivery donepezil active agent composition is provided. Aspects of the compositions of the invention include a donepezil active agent layer that is formulated to provide for multi-day delivery of a therapeutically effective amount of a donepezil active agent to a subject when the composition is topically applied to the subject.
