A business form includes in a first embodiment a wristband that is provided with a laminate backing including a tab portion at the opposite end of a free end, with the tab portion having a cinch slot through which the free end is inserted for securing the wristband about a person's appendage. The cinch may be located in one of several locations with respect to an imprint receiving area. Several individually separable self adhering labels may be provided inboard of the tab. The labels and the wristband may each be imprinted with identifying indicia such as bar coding. A second embodiment of the invention includes a page sized form with one or more self laminating wristbands readily separable from a carrier, and alternatively with a plurality of self adhering labels. The wristband may alternatively comprise an oversized laminate layer for laminating a face stock area for receiving an imprint, with a strap at one end for insertion through a cinch slot.
L'invention concerne un imprimé commercial comprenant, dans une première forme de réalisation, un bracelet pourvu d'une pellicule de protection stratifiée et qui comporte une partie languette placée à l'extrémité opposée d'une extrémité libre, ladite partie languette comportant une fente d'attache à travers laquelle on insère l'extrémité libre afin de fixer le bracelet autour d'un membre d'une personne. On peut placer l'attache à plusieurs emplacements par rapport à une zone de réception d'impression. Plusieurs étiquettes autocollantes séparables individuellement peuvent être prévues à l'intérieur de la languette. Les étiquettes et le bracelet peuvent être marqués chacun à l'aide d'inscriptions d'identification telles qu'un code à barres. Une deuxième forme de réalisation de l'invention comprend un imprimé de la taille d'une page, qui comporte un ou plusieurs bracelets autostratifiés pouvant être détachés facilement d'un support ; dans une autre forme de réalisation, l'imprimé comporte une pluralité d'étiquettes autocollantes. Le bracelet peut comprendre, dans une autre forme de réalisation, une couche stratifiée surdimensionnée qui se stratifie à une zone de film à étiquette destinée à être imprimée, et comporte à une extrémité une lanière s'insérant dans une fente d'attache.
