A cancer prognostic having particular utility in the prognosis of head and neck squamous cell cancer, in which the expression levels of either or both of Transforming Growth Factor Alpha (TGF-.alpha.) or Epidermal Growth Factor Receptor (EGFR) are assayed directly from a sample of tumor tissue. The expression level once quantitated is normalized as to standard, and the standardized expression level is compared to the prognostic threshold of about 83 % of standard for TGF-.alpha. or of about 23 % of standard for EGFR, or the corresponding upper threshold of the "low" tertile regardless of how calculated. Virtually all if not all patients demonstrating this low expression level survive at least five years after initial diagnosis, assuming completion of treatment with standard surgical tumor excision and radiation protocols for squamous cell head and neck cancer. Whether an individual patient's expression levels of TGF-.alpha. and EGFR fall inside or outside of this category signifies whether the patient is in a good or poor prognostic category, respectively, which in turn guides appropriate choice of therapy.
La présente invention concerne un pronostic de cancer convenant particulièrement dans le cas de carcinomes au niveau de la tête et du cou. On réalise en l'occurrence ce diagnostic par des évaluations des niveaux d'expression, soit du Facteur de Croissance Transformant .alpha. (TGF-.alpha.), soit du récepteur du facteur de croissance de l'épiderme (EGFR), soit des deux. Après évaluation quantitative, le niveau d'expression est normalisé par rapport à un standard, ce qui permet de comparer le niveau d'expression standardisé à un niveau plancher de pronostic d'environ 83% du standard pour le TGF-.alpha., ou d'environ 23% du standard pour l'EGFR, ou à un plafond correspondant du tertile "inférieur", indépendamment du mode de calcul. Virtuellement tous, sinon tous, les patients restant dans ce faible niveau d'expression survivent au moins cinq ans après le diagnostic initial, à condition qu'on ait mené à terme un traitement impliquant l'excision chirurgicale standard de la tumeur et les protocoles radiothérapiques pour les carcinomes de la tête et du cou. Le fait de savoir qu'un patient présente des niveaux d'expression du TGF-.alpha. et de l'EGFR appartenant ou n'appartenant pas à la bonne catégorie permet d'établir un pronostic vital optimiste ou pessimiste suivant le cas, et partant de mieux faire les choix thérapeutiques appropriés.
