A form assembly (12) for casting a lining wall in an ex- isting manhole (10) includes a cylindrical lower form assem- bly (28), a circular horizontal form assembly (32), arid a cylin- drical upper form assembly (30). All three assemblies (28, 32, 30) are formed of smaller segments (34, 52, 78) which are as- sembled together within the manhole (10) and which can be disassembled after the manhole (10) has been repaired.
