Conduits are provided to direct blood flow from the left ventricle to a coronary artery at a location distal to a blockage in the coronary artery. Threaded and nonthreaded conduits are delivered using a guidewire delivered through the pos- terior and anterior walls of a coronary artery and into the heart wall. A dilator may be provided over the guidewire into the heart wall, and the conduit delivered over the dilator. An introducer sleeve may be provided over the dilator into the heart wall, the dilator removed, and the conduit delivered through the introducer sleeve. A hol- low needle also may be inserted into the posterior and anterior walls of the coronary artery prior to inserting the guidewire. A depth measuring tool may determine the appropriate length of the conduit prior to delivery. The depth measuring tool can in- clude the hollow needle with an acces port on a proximal end of the needle and an opening on the distal end of the needle in flow communication with the access port so that when the needle is inserted through the heart wall and into the heart chamber, blood flow through the opening.
Cette invention concerne des conduits pratiqués pour diriger le flux sanguin du ventricule gauche vers l'artère coronaire en un point éloigné d'un blocage dans ladite artère coronaire. Des conduits, filetés ou non, sont mis en place au moyen d'un fil de guidage que l'on fait passer au travers des parois postérieure et antérieur de l'artère coronaire et dans la paroi du coeur. Le fil de guidage peut être muni d'un dispositif dilatateur, et le conduit être implanté par dessus ce dilatateur. Un manchon d'insertion peut être glissé sur le dilatateur dans la paroi du coeur, à la suite de quoi on retire le dilatateur et on fait passer le conduit dans le manchon d'insertion. Une aiguille creuse peut être introduite dans les parois postérieure et antérieure de l'artère coronaire avant insertion du fil de guidage. On peut utiliser un outil de mesure de la profondeur pour déterminer la longueur appropriée du conduit avant sa mise en place. A cet outil de mesure de la profondeur peut être intégrée l'aiguille creuse, laquelle comporte un orifice d'accès sur son extrémité proximale et sur son extrémité distale une ouverture en communication fluidique avec l'orifice d'accès, de telle sorte que le sang s'écoule par ladite ouverture lorsque que l'aiguille est insérée au travers de la paroi du coeur et dans la chambre cardiaque.
