This invention concerns novel formulations for opioid-based treatments of pain and/or nociception comprising opioid analgesics and 1,4-di-piperidin-4-yl- piperazine derivatives having neurokinin antagonistic activity, in particular NK1 antagonistic activity, the use of said formulation for the manufacture of a medicament for the prevention and/or treatment of emesis, pain and/or nociception, in particular in opioidbased acute and chronic pain treatments, more in particular in inflammatory, postoperative, emergency room (ER), breakthrough, neuropathic and cancer pain treatments and the use of an NK1- receptor antagonist for the manufacture of a medicament for the prevention and/or treatment of respiratory depression in opioid-based treatments of pain. The pharmaceutical formulations according to the invention comprise a pharmaceutically acceptable carrier and, as active ingredients, a therapeutically effective amount of an opioid analgesic and NKI -antagonists according to the general Formula (I) the pharmaceutically acceptable acid or base addition salts thereof, the stereochernically isomeric forms thereof, the N-oxide form thereof and prodrugs thereof, wherein all substituents are defined as in Claim 1. The pharmaceutical composition according to the invention reduces to a large extent a number of unwanted side~effects associated with opioid analgesics, in particular emesis, respiratory depression and tolerance, thereby increasing the total tolerability of said opioids in pain treatment.
La présente invention concerne de nouvelles formulations pour les traitements à base d'opioïdes contre la douleur et/ou la nociception comprenant des analgésiques et des dérivés 1,4-di-pipéridin-4-yl-pipérazine présentant une activité antagoniste de la neurokinine, notamment une activité antagoniste NK¿1?, l'utilisation de ladite formulation dans la préparation d'un médicament destiné à la prévention et/ou au traitement des vomissements, de la douleur et/ou de la nociception, notamment dans les traitements contre la douleur chronique et la douleur aiguë à base d'opioïdes, plus particulièrement dans les traitements inflammatoires, postopératoires, en salle d'urgence, de métrorragie, de la douleur neuropathique et liée au cancer et l'utilisation d'un antagoniste du récepteur NK¿1 ?pour préparer un médicament destiné à la prévention et/ou au traitement de la dépression respiratoire dans les traitements de la douleur à base d'opioïdes. Les formulations pharmaceutiques selon la présente invention comprennent un vecteur pharmaceutiquement acceptable et, en tant qu'ingrédients actifs, une quantité thérapeutiquement efficace d'un analgésique opioïde et d'antagonistes NKI représentés par la formule (I), l'acide ou les sels d'addition de base pharmaceutiquement acceptables de ce dernier, ses formes stéréochimiquement isomères, la forme <I>N-</I>oxyde de ce dernier et les promédicaments de ce dernier, tous les substituants étant définis dans la revendication 1. La composition pharmaceutique selon l'invention réduit dans une grande mesure une pluralité d'effets secondaires indésirables associés aux analgésiques opioïdes, notamment les vomissements, la dépression respiratoire et la tolérance au médicament, ce qui accroît ainsi la tolérabilité globale desdits opioïdes dans le traitement de la douleur.
